If you haven’t already done so, please contact our office at 949-727-1772 or fill out our electronic form, in which case, we will contact you to set up an appointment that works best for you.
Patient intake forms
Once you contact us for your appointment, we will send you all pertinent patient forms to be filled out prior to your first visit. It is important to complete these forms to the best of your abilities so that all information is complete. If any information is left blank, you will need to complete the form on the day of your visit. For this reason, we ask that you show up at least 15 minutes ahead of your scheduled appointment to complete and turn in your patient form.
The day of your appointment
We ask that you arrive early so that we can double-check all your intake forms for accuracy. In cases where you’ll be paying with insurance, we’ll need that extra time to figure out what benefits are covered and discuss payment options ahead of time.
We also request our patients to show up in comfortable loose fitting clothing and sneakers. Chiropractic care does require the doctor to move you around at times, so being comfortable is key to having a pleasant experience.
Once you are called in, you’ll be meeting with one of our chiropractors and go through an extensive examination. This exam typically involves taking digital x-rays of the spine, assessing your posture, and asking questions about areas of pain and discomfort. If you happen to have previously taken x-rays or imaging, please bring them with you. Having access to your medical history is important to determine your overall health and how we can best plan our treatments for best results.
What follows the initial exam and consult?
Upon completing the initial exam and consultation about the state of your spine, you are given options as to how you wish to proceed. Typically most patients are ready to be adjusted and receive their first course of treatment. In some cases, patients would rather think about next steps. Whichever you’re comfortable with, we highly recommend being prepared for some degree of treatment especially for patients who are experiencing severe pain. Getting an adjustment typically alleviates pain quickly, and provides temporary relief until a full decision has been made regarding the overall course of treatment.
